You can send email notifications on behalf of any email address that is added as an alias in your Gmail account.

You should create three separate rules in the Google Forms addons and keep the email body different for each of the rules.
Also, each rule should include a condition that should be something like “Venue Name Question” equals “Location”.
Each rule would thus only be executed if that venue in the condition matches the location name.

You can download a fully working version of Document Studio from the GSuite Marketplace. It is free for basic use.
You need to go to your Gmail account settings and add an email alias as shown in this tutorial.
Please ensure that you have turned on the “Treat as Alias” option.
Do you have multiple Google accounts? Can you check if you have enabled sending from a different Google account than your main account?
If this doesn’t help, please go to the Document Studio menu inside Google Sheets and send me the debug logs.

You only need a single license for the user who would be using Mail merge. It doesn’t matter how many users are in your GSuite organization.
A user is the same as a Google Account. You only need a license for the person who is sending the email, not for the person who is editing the document.
However, the person who is editing the file will not be able to send emails using your license. The emails can be sent only by the user who has the license though they can send on behalf of any email address that is available as an alias in their Gmail account.

Sorry, it is currently not possible to delete the execution of Document studio triggers.
An alternative, you may use the Merge every hour option and it will merge all the new form responses received in the last hour in one go.
If your script function processes the form data, you may consider using ARRAYFORMULAS that run almost instantly when a new row is added to the sheet and thus available to Document Studio when generating the PDF document.
